Connect Together

Connect Together is a social prescribing service for children and young people.
It helps them find support to improve their wellbeing and physical health.
The service also helps with school readiness and reducing loneliness or social isolation.

Where It Is Available

Connect Together is available for children and young people who live in LS12, LS13, and LS28.
It is also for children and young people who are registered with:

  • GP surgeries in the Armley Primary Care Network, or
  • GP surgeries in the Beeston, Middleton, and Hunslet Primary Care Networks.

Disability: Connect Together supports children and young people with any disability.

Age:  5 to 16.d
